Onward Transitions, a therapeutic program for young adults aged 18-30+ in South Portland, Maine.
Program Overview:
What it is: A non-residential, clinically sophisticated Intensive Outpatient Program (IOP) focused on kindness, acceptance, affirmation, and autonomy.
Location: Resource-rich Portland, Maine, chosen for its coastal location, proximity to major cities, and natural areas. This small city of 75,000 has vibrant food, arts, music, and sports scenes, as well as plentiful jobs, educational options, and social recreational opportunities.
Approach: Staff are trained in the developmental stage of emerging adulthood, coaching executive functions, and primarily use an ACT (Acceptance and Commitment Therapy) model in therapy.
Goal: To empower young adults to become "the best versions of themselves" and achieve greater autonomy, meaningful relationships, and impactful contributions.
Program Details:
Participants (Members): Work with us to find apartments and build treatment plans (of up to 20 hours per week). They are described as kind, accepting, affirming, neurodivergent and neurotypical, socially motivated, and looking for community. They have mild to moderate symptoms of anxiety, depression, and executive function challenges, and are committed to using their prescribed medication. Some have completed primary treatment, while for others, OT is their highest level of care.
Therapeutic Services: Five full-time therapists provide individual and group therapy, and personalized family work. The weekly schedule includes 16 clinical and coaching groups, 10 shared meals, and community exploration.
Staff: Six full-time, benefited employees focused on improving members' lives, acting as allies rather than "power brokers."
The Pine House: Our over 4,000 sq. ft. central location is staffed weekdays from 10 am to 6 pm, with 24/7 on-call support for emergencies. The Pine House has private office spaces, two communal kitchens, a member coffee house, multiple all-gender restrooms, including a shower, a large great room for group activities, and a green space with ample parking.
Length of Stay: Members typically start with a year-long apartment lease, though some may transition earlier. Most stay beyond 12 months, and over half remain in Portland afterward.
Life in Portland: Portland is a small, coastal, safe, progressive, walkable city with public transportation. It's resource-rich, with colleges, arts, an award-winning food scene, a bountiful job market, and diverse recreational opportunities. Members usually live in downtown studio or one-bedroom apartments.
Fees: $12,000/month, covering all clinical and coaching services, 24-hour on-call, 10 meals/week, social activities, limited corporate Uber, welcome/move-in kit, apartment consults, limited shuttle, Google Workspace, and World of Work Inventory (WOWI).
OT Community: Staff foster a supportive peer culture where members build relationships, engage in social activities, and feel understood by like-minded individuals.
